Student Info Sheet, Medical Lab Form/Student Safety Contract went home on the first day of school. Students cannot participate in lab without these filled out and returned. On the parent notes letter is a weblink to fill out student information.It is also located in the Favorite Links at the bottom of this webpage. If you have no internet access, please have your student ask for a hard copy to fill out. Please fill these out as soon as possible. Class fee is $8. Students must pay fee or make payment on fee to begin labs. They will have an alternate assignment if fees not taken care of or lab medical form not completed and turned in. I purchase lab supplies, paper towels, kleenex, sanitizer, wipes with this fee. Students need a binder for science or shared with another subject. One section of the shared binder must be for science only, organized with tab dividers labeled as follows: Learning Log, Handouts. Students may write in pencil only. NO INK PENS ARE TO BE USED. Students need pencil colors for science activities.
