My child, ______________________ has permission to stay after school on

Tuesday, November 7th to practice with the chorus for the Veteran’s Day program from 2:45pm-4:30pm. I will promptly pick my child at 4:30pm. If my child is not picked up in a timely manner, my child will be given one more chance to remain in chorus, and then will be cut from the group if not picked up on time. Our program for Veteran’s Day will be on Thursday, November 9th from 8:35am-9:05am, 9:10am-9:40am during the school day, and 6pm in the evening

I will promptly pick my child at 4:30pm. Greetings from the MBE Music Department!

Our Jr. and Sr. Chorus will be taking a field trip to the Spring Music Festival at First Baptist of Orange Park on Kingsley Avenue. If you are interested in participating as a chaperone, please detach the form below and send it in with your child no later than Monday, May 8th. I will need to have a least 15 chaperones participating in this trip in order to take it. I currently only have 3 volunteers. If I do not have any other volunteers, we will not be able to go on the trip this year.

Please send in your form with your child’s permission slip and money as soon as possible in order to confirm your seat on the bus, if you wish to ride on the bus.

EVENT: The Annual Clay County School District Spring Music Festival

WHEN: Tuesday, May 16th—permission slip and money due on Monday, May 8th

DESTINATION: First Baptist Church of Orange Park

Address of church: 1140 Kingsley Avenue, Orange Park, FL 32073

WHAT TIME: Buses will depart MBE at 9:30am. The program will begin at 10:15am. Students will return to school around noon or 12:30 (approximately). We will have Little Ceasars pizza on school grounds.

COST: Admission to this program is FREE! The only cost is $5.00 for bus transportation via the county. The cost for parents to ride the bus as a chaperone is also $5.00. Personal checks should be made out to MBE with “Chorus Field Trip” in the memo. Students must pay for cost of pizza and water ordered on the field trip day.

DRESS CODE: Students are REQUIRED to wear a red t-shirt. Students who do not come to school with a red shirt will not be able to attend the field trip, even if they have paid and turned in their permission slip. Students should wear red t-shirt with jeans, and black shoes. No high heels or high heeled boots girls.

DUE DATE: Students and parents bus money, pizza order, permission slips, and parent Volunteer Registration forms are all due on Monday, May 8th. Monday, MAY 15TH will be the last day to turn in $$ & permission slips. If they are not all submitted by or before this date, your child will not be allowed to go on the field trip. If your permission slip and money are turned in this late….you may not get pizza.
